Protasis: εἰ with the imperfect indicative.

Apodosis: ἄν with the imperfect indicative.

εἰ ταῦτα ἐποίεις, καλῶς ἂν ἐποίεις if you were (now) doing this, you would be doing well; if you had been doing this, you would have been doing well.
